When children join our school or nursery, we ask parents to provide us with a range of information about your child and yourself. This includes contact details so that we can always contact you or another named person in the event of illness or an emergency. It is vital that you keep us up-to-date if your details change e.g. a change of address or a change of mobile telephone number. The form below can be downloaded and completed to keep us informed of any such changes. Your support with this is much appreciated.
We also ask parents to give consent for a range of activities that take place in school. This includes activities such as your child having their photograph taken, taking part in visits within the local area and making use of the school's internet access. All parents are asked to complete a consent form when your child joins our school and this lasts for the entire time that your child is with us. You have the right to withdraw your consent for any activity at any time - the consent form can be updated by downloading a copy below.
